1. 居家個案開案、簽約、服務派案。
2. 個案異動評估及家屬溝通連結。
3. 協助居督安排居服員人力調派、溝通與協調。
4. 個案所需資源協調與轉介。
5. 監督居督服務品質與審核相關服務紀錄。
6. 需有長照LEVEL1
