Intake Coordinator Qualifications

At least 3 years of hospice experience (intake preferred, billing acceptable) in a high-volume referral setting Receive and promptly process hospice referrals and inquiries, scheduling a hospice representative with the patient and family to assess the need for services Assign patients to the appropriate team after admission, in coordination with team leaders Ready to apply hospice experience to the workflows of a centralized system; central office experience is a plus, including handing off and receiving incomplete work items Strong working knowledge of Teams, SharePoint, and electronic clinical records Able to work across multiple systems and juggle competing priorities with strong attention to detail Compassionate, professional phone presence Reliable transportation, a valid driver's license, and the ability to safely operate a vehicle Intake Coordinator Schedule:
