Duties

• Patient assessment : evaluating conditions, mobility, strength, and pain levels.
• Reviewing medical history and collaborating with other team members.
• Identify functional limitations and rehabilitation needs.
• Treatment planning: develop individualized rehabilitation programs based on diagnosis and team input.
• Set short – and long- term goals.
• Adjust plans depending on patient progress and interdisciplinary recommendations.

Therapeutic Interventions :

• Provide exercise therapy, manual treatment and modalities.
• Patient and Family education: teach patients proper exercises and injury prevention strategies.
• Educate families on supporting recovery and committing to prescribed plan of care.
• Promote long-term health and independence.

Monitoring and Documentation :

• Track patient progress and outcomes.
• Maintain accurate clinical records to be completed prior to end of shift.
• Modify interventions based on results and team feedback.

Qualifications

Physical Therapy license in NJ Graduation from an accredited Physical Therapy program (DPT preferred, not required) Schedule:
