Nurse Practitioner Johnstown PA Subacute Rehabilitation & Post-Acute Care | Part time | $80–$90/ hour We are seeking a compassionate Nurse Practitioner to provide on-site care at subacute rehabilitation facilities in Johnstown and Hastings, Pennsylvania. This part-time position requires one provider to cover both locations. Join a supportive team helping patients manage pain, improve mobility, and regain independence.

What We Offer $80–$90 per hour , based on experience Part-time, flexible scheduling No nights or weekends Strong administrative support and streamlined scheduling Laptop provided Malpractice insurance coverage, as applicable Responsibilities Complete comprehensive patient assessments and follow-up visits in subacute rehabilitation settings. Manage rehabilitation-related medical needs, pain, and conditions affecting mobility and function.

Develop individualized treatment plans in collaboration with the supervising or collaborating physician and facility care teams. Prescribe and manage medications within your scope of practice. Perform joint and trigger-point injections when appropriately trained and authorized.

Coordinate with physical, occupational, and speech therapy teams to support patient progress. Maintain accurate, timely clinical documentation. Educate patients and families and assist with care transitions.

Qualifications

• Current Nurse Practitioner certification Active Pennsylvania RN and CRNP credentials Active DEA registration for prescribing controlled substances Experience in subacute rehabilitation, geriatrics, orthopedics, internal medicine, pain management, or a related setting Strong clinical assessment, documentation, and communication skills Ability to manage a patient panel and collaborate effectively with interdisciplinary teams Availability to provide on-site coverage in both Johnstown and Hastings Help patients move forward in their recovery while enjoying a schedule that supports your work-life balance.
